His father's birthplace was noted as Donegal County, Ireland. The name of his mother, Margaret McGonigle, was given as his nearest relative, her address being the same as his own. He signed his name. The details of physical appearance that were recorded were 'eyes: light blue' and 'hair: auburn'.
